Add Participant

Please note: Once a participant's information has been submitted an email will automatically be sent to the participant. This email will contain a link to the survey login screen, the username that was assigned to the participant and, an automatically generated personal password. It is to this email that participants should refer when they are attempting to login to the survey site. (If the email is going to an AOL.com address, AOL may mistakenly identify the email as SPAM and place it in the recipient's SPAM folder.)
